Flu vaccine for Maori and Pacific People

This year the flu vaccine is funded for all Maori and Pacific people aged between 55-64. If you are in this age group and would like to have a flu vaccine please phone the receptionist to book an appointment.

Enrolments accepted based on Practice capacity and residential address ie you live in our geographical zone

Please fill in the form, then print it off and hand it into our reception
